You are due to complete your required paperwork before your appointment. Please bring a photo ID, current insurance cards), and an updated medication list. If paperwork is not completed in advance, arrive 20-30 minutes early to complete it in the office. Incomplete paperwork may result in cancellation or rescheduling.
